Are you using Totara version 9 or above - iCal attachments are not included in Totara v9 and above - they are included inline in the message.
Can you provide a copy of the email received by the supervisor?

I can't see anything in the confirmation notification that is able to be turned off for notification message for the manager iCal. There only seems to be a global setting to disable iCal for seminars.
I can't think of a good way around this - possibly unticking the 'Send copy of this notification to the user's manager' in the confirmation notification and relying on the information sent to the manager from the Seminar booking request notification.
