Quiz not showing in Module

I have multiple modules set up with links to videos followed by a quiz at the end. However, several individuals (but not everyone) are not able to see the activity with an error message that comes up saying "Sorry, this activity is currently hidden." Currently, of about 300 individuals who have completed the module, only about 10 are not able to view it. We assigned both modules as certifications, but with one of them having two main audience groups to encompass everyone, the other assigned to specific staff in specific departments.

Everything I can find in the settings for the Quiz, Course, and certification is showing that it's visible. What am I missing?

Some ideas on what might be causing this

What course format have you got - have you got Topics format - or Single Activity Format - change to Topics as the single activity format only supports one activity.
If you have topic format is one of the topics hidden?
Do you have any Restrict Access in the video activity?
Do you have audience visibility turned on and the user is not a member of the audience?

Thanks for your reply. I have a topics course with three sections, all three are visible, nothing is hidden, and there are no restrictions on the video activity. By "Audience Visibility", do you mean the actual activity being visible (vs. hidden)?

Ok that eliminates most possibilities - the audience visibility question you can ignore as this applies on a course level rather than activity level.

Are you able to create a helpdesk ticket and attache a backup of the course without user data so we can see if we can see what is causing this.
