If I needed to update a course's content, add or remove activities and update completion criteria and that course was part of a certification would bad things happen to users on the recertification pathway or can Totara keep track of all of that.

Totara does keep track of this as follows.
For existing users the certification will remain as complete and when the recertification window opens for the user they will need to complete the newly changed recertification pathway
For newly assigned and in progress users they will need to complete the certification based on the new criteria.
