It isn't possible to add questions to a quiz that has had an attempt recorded

What you could do is Add a question to a question bank category and then if you add a random question from the category the new question could be selected. If you already have set questions then not sure how you could get around this.

The reason for this is to ensure the consistency of results - so two people don't have same results for different quizzes.
