I wasn't able to locate an edit function that enabled me to modify/add these fields to the Feedback Responses Report?

This report is a legacy report that is not available to edit with the report builder.
The only report builder report is the Feedback summary which doesn't provide question analysis.
Totara Perform does include performance activities that can be created and assigned to specific audiences. These include different types of questions and also question response reporting using the report builder.
