We have a request from a customer which is currently not available. 

When a learner tries to add a new evidence item, the user is able to save the evidence without giving a name. The system combines learner's name and evidence type name and adds in the evidence name field when we save the data. This is not ideal as there are scenarios where users will be using the same evidence type multiple times. It'll be hard for administrators to run reports and check credibility.
