Probably overthinking this.
We need to set up some complex combinations of Programs and Certifications:
For example:
A program of courses need to be assigned and several of those courses also need to be assigned to certifications for those who complete them (individually or as part of the program).
Is there any possible issues caused by doing this?
The only issue I can think of is where a Program contains more than one course (so completing the course doesn't immediately complete the Program), and the user is also enrolled in a Certification containing that course which they complete and the refresh window opens (resetting the course). In that situation, the course would need to be completed again (along with the other courses in the Program) for the Program to be marked as complete.
If the audience rule for enrolling a user into the Certification is that they've initially completed the Program, can't see any issues there.
Like Daniel said, this is pretty low risk and is a very common use case. Especially when it comes to new starters who complete an induction program that also contains certification courses.

I think I understand your points and how that relates to my situation - given that the Certification is based on the Courses and not the Programs, this might cause an issue with Programs never being complete, or by them being reopened each time the Certification window reopens(?)
Think I'll be better to just assign the individual courses/certifications via Audience and assign Due by Dates, as there is no specific order they need to do them.

Just remember that once a program is complete for a user, it won't be marked incomplete by a course resetting (as part of a certification) or even adding a course to the program. It's actually based on course sets, so if a program had multiple sets that need to be completed in order, once a user has completed the first set and new content is added to the first set, they wouldn't even need to go back to complete that.
