We have a category that does not contain any programs or courses, yet I am not able to delete it. Other categories come up with a Delete option when I click the cogwheel icon on the Courses and Categories page, but this option doesn't come up for this one.
What could be the reason?

Do you have multi tenancy enabled and the category is associated with a tenant - it isn't possible to delete this category unless you delete the tenant (only if deleting the tenant is what you want to do)

Hm, I'm unsure if we ever had tenancy enabled for this instance (I don't think we ever had), but now it's definitely off: both Enable multitenancy support and Enable tenant isolation are off.
I noticed that categoryid=1 in the url. Can it have been the first category and so cannot be removed?

I think that is right the category id 1 is reserved for the site level category usually Miscellaneous that is prevented from deletion .
