This could be caused by a custom theme not functioning properly. If you have a custom theme then select a standard Totara theme and see if this fixes this - if it does the custom theme is probably the culprit and you will need to go to whoever developed this.

If this is OK then can you provide us with what version of Totara and browser you have.

Thanks for the screenshot you sent me - you are right when selecting the user can manually complete the activity for a label results in the checkbox being misaligned to the activity below.

I have reported this as a bug to be fixed.
