DDL sql execution error when activating appraisal

Hi, 

When I try to activate the example appraisal, it works as it should. 

So I duplicated the example appraisal and modified the questions to my client's needs, but when I try to activate this appraisal I get the "DDL sql execution error".

Does anyone have an idea why?
