Totara Learn Open Discussions

We are running R11.  Have a Program with five 3-topic courses.  All courses have completion tracking.  Need to complete sequentially so courses #2 through #5 are not availble until the previous course is completed.   An audience is assigned to just the program.  All courses are visible to enrolled users.  When first opening the program, and clicking "Launch", the user is automatically enrolled.  When the user completes course #1, the Program shows a partial completion and course #1 is on the completed learning.  The "Launch" button becomes available on course #2, and the user clicks it.  The user does not get an enrolled message, the user does not show enrolled in course #2, and the course does not show in My Learning.  When the users completes course #2, there is no progress for the course, it does not show in completed learning, and course #3 is unavailable.  All courses are set up the same way with the same settings.  Any ideas why the auto-enroll only works for the first course in the program?
