I set up an evaluation form in our Totara 12 system, tested it (together with my colleagues) and all worked well. I did some instructions and have asked colleagues to add this to their courses from a template.
They have done this however now it is not following correctly, please see attached.
Those who have chosen classroom and entered location should be jumping on to pre-course information however they are getting the response to text rating poor (highlighted on the attached... We are sorry your experience was poor.). I have looked at this and cannot see any reason for this to be happening as nothing has changed in the form.
I have tried putting breaks between the questions but that has not helped. I would really appreciate any support from anyone on this forum.
