Hi Jason

There is a reset to default button under

Site Administration -> Users -> Permissions -> Define Roles. Select a role name and then press the Reset to defaults button.

Hopefully this will sort this out - if not then check that the role names are assigned to users at the correct level eg course level for learners usually.
