There is a Moodle 1.9 block here that should be compatible with Totara:

That is a block that you can include on pages, but it won't replace the calendar date pickers used throughout Totara.

They should show the days in arabic if your servers locale is installed correctly but they won't use the alternative date system of the Ummul Qura.

Unfortunately I don't think that's something that we can implement - you would need to modify it to do that yourself as a local customisation. If you (or another client) was able to make the changes we'd certainly be happy to consider including it in core (as long as it could be enabled via an option).
