Just wondered if anyone else has experienced this?
We have a small number of users who complete the activities and receive a grade but the completion status is "not yet started" so its not completing their course.
I can't see any obvious pattern to it.
This is on Totara 2.5.8

Just a thought, maybe it is the old messaging transaction bug? If an activity is set to send out some sort of notification email when it is completed then this can break the completion process. We fixed it for quizes in 2.5.13.1 but there may be some other scenarios with other activities that we don't know about yet.
