Is it possible to use the same questionnaire, survey, or feedback activity within multiple courses? A few of our courses at this time, require the user to complete a Microsoft Form so that way all of the survey data is on one spreadsheet.  If we wanted this data to be housed in the LMS, is there an activity that could be used across multiple courses and work the same way as a Microsoft Form?

If the data are ok in one spot, the database activity might help. However if the data is required in individual courses and the answers are yes/no, a sneaky way is to use a quiz with binary weighted grades. The resulting grade represents a unique combination of responses.

We use the Questionnaire activity, and have it stored on the main site pages, rather than a specific course (as the help doc suggests): Setting up a questionnaire - MoodleDocs


We then export the data and use Excel/PowerBI for additional reporting.

Hi Candace,

If you create a feedback activity on the front page of your site, you can use it across multiple courses.

We use this to collect feedback about our Totara Community courses, and we are then able to compare feedback scores across multiple courses.

Yes, I agree. We did this and it worked well.

Many years ago I used the Feedback activity to enable people to pre-order their lunch-time baguettes from the canteen and choose their filling, :-). We used a course page to support the Canteen, rather than use an alternative intranet, to encourage staff engagement.

This possible solution feeds directly into a need we have now.

Could you have this established on the main dashboard instead of on the front page?  We use our front page as a public landing page so it's not exactly working the same in the tests so far and I'm wondering if that is a factor.

The courses where we would use this global feedback form awarded a certificate of completion once the user completed the feedback form in the course.  With the feedback now being collected elsewhere - and the new feedback activity being a URL where they get credit for viewing - how could we base the certificate generation on them completing the global feedback activity?

Hi Martha,

We have the same issue with our front page being the landing page for guest users in the community.

This is the front page that guest users see.


In order to add the feedback to the front page and keep it hidden, you can follow these steps.


1. Edit the front page settings and turn on 'Include topic format'


2. Turn editing on

3. Add your feedback activities to the front page. Your page might now look something like this: 


4. Bookmark, or make a note of the URL's for any feedback activities you create. (Otherwise it can be difficult to navigate back to these).

5. Edit the front page settings again, and not turn 'Include a topic section' off. The activities you added to the topics section will still exist, but will not appear on your front page anymore.


How could we base the certificate generation on them completing the global feedback activity? 

For your second question - I don't think there is a way of making sure that users answer the feedback. 

The closest thing you could do here would be to require the learner to view the URL for the activity to be marked as complete.

They will then need to open the feedback, but they wouldn't have to submit anything to receive the certificate.
