Hi Gareth,
How were the users created? Can you create a test user by the same method and see what is being sent out to them by way of username & password? How are they accessing Totara?

In addition to George's questions, are you using any specific authentication plugin (other than manual) that could be the source of this problem?
You might also find some diagnostic information in the web server log.
