Another possibility is this might be able to be done using learning plans. You could set this up the questions as  competencies or objectives and set them a priority scale of achieved or not achieved.

The workflow setting in the learning plan could be set so that only managers can change this. This would also be accessible easily the My team menu for the manager.

A report could be created in the report builder using the learning plan report source that shows the managers priority scale values.

To get a total score would require the report to be exported as the report builder currently can't do this.

We’ve also had this issue and went down the path that Craig has suggested. Our focus was around ensuring alignment with L&D solutions to ratings in core tasks and integrating with our just launched use of the Totara development plan process.

We had built a new section called Core Tasks (we’re using the Workware methodology from AOM) where Team Leaders can add Tasks being processed by their team and then rate staff to a scale.

We’ve made it colour coded for quick visual assessment and the pop-up rating box pre-loads the next record so it appears instant.  The Core Tasks are competencies but suppressed from view of Competencies by staff.

In the screen shot I haven’t shown that the rating box also shows the linked Competencies and completion of supporting development opportunities. You can, however, see the Comment links – this and the core task appear on the staff member’s development plan which is how we tie all these things together.

Glad you have got the learning plan set up as you want it.

The administrator would be able to create a learning plan template that has everything set up as the manager required.

Learning Plan > Manage templates

This template could then be made the default template and then each of the manager's staff can create a learning plan based on the learning plan template.

It may be better for the admin to create the learning plans so that others using the system don't have the opportunity to use the default template.

There is a feature request to make the selection of the learning plan template possible rather than having just the default learning plan template available.
