This is a known bug and was fixed in v2.2.18, upgrading to the latest 2.2v should fix the problem.

Yes - the program completion runs overnight. Incidentally - Totara 2.5 will have instant course completion, and instant program completion.

Apologies for the delay, somehow we missed this one.
Can I ask you what value is in the Grader report?
If there is nothing then run this SQL on your database "SELECT userid, itemname, rawgrade FROM mdl_grade_grades gg JOIN mdl_grade_items gi ON gg.itemid = gi.id WHERE userid = X AND courseid = Y;"
This will tell us where the issue is happening.
