Have a look at the documentation  "Working with Performance Management" at page 124 for how to assign users that the feedback form is being completed about. It includes a walkthrough and screenshots.

The assigned Learners and their Managers can then request that the feedback form be completed by users using the steps at the bottom of page 177.
